Most PDF readers offer a rotate button that turns the on-screen view without changing the document. Close and reopen the file and it is sideways again. Saving through this tool writes the rotation into the file itself, so it travels with the document.
Scanners record orientation inconsistently
A sheet-fed scanner may mark alternating pages as rotated when the physical pages were fed the same way. This is why a scan often needs different rotations on different pages rather than one value applied to all.

Rotation is a page attribute, so writing it costs nothing
A PDF page carries a rotation value alongside its content — 0, 90, 180 or 270. Turning a page permanently means writing that value into the file, not re-drawing anything. Nothing is re-rendered and nothing is re-compressed, so a rotated page is byte-for-byte as sharp as it was, however many times you turn it. Flipping and rotating are not the same operation
A good share of this cluster asks about flipping a PDF. Usually that means rotating — turning a landscape scan upright, or putting an upside-down page the right way up with 180°, both of which this does. Alternating upside-down pages come from duplex scanning
